What is a Continence Assessment?
A continence assessment is a process where a Continence Nurse Specialist evaluates your condition to identify the causes of incontinence. With their expertise in bladder and bowel management, our nurse specialists will create a care plan tailored to your needs. This plan will help guide you through managing urinary and bowel incontinence effectively.
The Continence Assessment Process
Your continence assessment will typically take around 1.5 hours. During this time, the continence nurse will gather crucial information to understand the nature of your condition, including your toileting habits, frequency of incontinence, and any other relevant medical details. You may be asked to fill out charts and possibly undergo urine tests for further insight into your condition.
Preparing for Your Continence Assessment
To help make the assessment as effective as possible, it's recommended that you maintain a bowel and bladder diary for a few days prior to your appointment. This will assist the nurse in understanding the full scope of your condition. Bringing along a list of your health conditions, medications, and any other relevant medical history is also a good idea.
Next Steps After the Assessment
Once your continence nurse has gathered all necessary information, we will create a tailored care plan for you. This plan may include product recommendations, training, and future check-ins. The assessment results, along with the care plan, will be compiled into a comprehensive report that can be shared with your family, health professionals, and support workers. This report ensures everyone involved in your care is on the same page.
